上海市世界外国语小学教学评价环节各项制度

一、教学评价内容、形式:

1、日常单元测验和练习由备课组长把好单元质量关,统一进度、统一测验时间。单元测试后组内要进行教学分析,发现问题及时补漏。

2、期中、期末考试(查)实行"教"、"考"分离,由校长室负责提供试卷,使任课教师增加目标意识,提高教学效益。

3、实行多样化评价手段,有笔试、口试、活动能力等多方面的考核。

(1)英语期中、期末口试在笔试前一周统一进行。

(2)一年级通过"学科游园"活动的形式,进行学生学习情况的评价。

(3)四年级英语分流统一进行期末考核,评价等第记录在"成长手册"中。

二、教师监考:

1、提前阅读监考时间表,了解自己监考的班级和时间,做好准备。

2、考试当天监考老师提前10分钟进监考教室(如果班主任是当天的监考老师,请副班主任提前15分钟进教室管理班级),确保监考老师不迟到。

3、考卷由校办直接分发。

*4、监考时老师不得将手机带入考场,或做与监考无关的事情,经常巡视。

5、考试时间要书写在黑板上,如果试卷有正反两面的要提醒考生。

6、监考老师要逐一核对学生的姓名是否已写。

7、考卷如果出现问题(或错误),由总监考进班级进行全班统一讲解修改,监考老师回答学生的提问不要轻声耳语,要让全班听见。

8、老师喝水、走路尽量要轻。

9、考试结束前15分钟要开始提醒学生时间,对做得比较慢的学生要予以督促。

10、收考卷时,先宣布考试结束停笔,学生坐在各自座位上,监考老师依次收齐考卷、文稿纸、草稿纸,清点正确后才能宣布考试结束。

(所有在考场中下发的试卷、文稿纸、草稿纸均要求学生在指定位置写班级、姓名、学号,考试结束时全部收上来。试卷收取方式,每组一叠,每班共8叠)

11、监考老师认真、完整填写"监考情况登记表",监考结束时和试卷一起上交。

12、监考老师将试卷和登记表交到统一地点,由负责老师收齐,统一装订并交至阅卷地点。

三、阅卷、誊分、改分

1、期中、期末考试(查)和校监控均采用学科大组集中阅卷,"流水批改"的形式进行。每个年级的老师不批改本年级学生的试卷。

2、学科大组阅卷听从教研组长安排,统一时间、地点。阅卷前由教研组长和备课组长共同对每个年级的批改标准进行统一,并安排阅卷人员的分工。如批改过程中遇疑难问题,及时反馈至教研组长,统一进行调整。

3、阅卷教师把好批改质量关,认真校对,减少差错。阅卷完成后,由阅卷教师当场认真誊写分数,确保上机的分数正确。并对各班的总分、平均分和各分数段的人数及比例进行登记,以便第一时间了解测试情况。

4、班主任需逐一复核上机分数,并签名确认。如果发现试卷的批改和分数有问题,任课教师和班主任不得擅自更改分数,改分流程如下:

(1)任课教师填写"学生成绩复核申请",并将此申请与问题试卷一并交由大组长(境外交副组长)进行复核。

(2)大组长(或副组长)复核后,进行改分登记,并在"学生成绩复核申请"上填写"复核结果"。

(3)最后由部门校长在"学生成绩复核申请"上签字确认,方可进行上机分数的修改。

(4)如发现自行改分,未上报的情况,学校将对相关人员进行处理。

5、分数上机后再出现问题的,相关人员要写出书面情况说明,学校酌情予以处理。

(请各位老师严格遵守考试制度,本着公平公正的原则做到:认真监考,负责批卷,细心誊分;不拿分数卖人情;不徇私舞弊;不改动分数;不对外泄露试卷等。若发现考试期间有任何违背师德,影响世外声誉的事发生,校方将追究相关人员责任,绝不姑且。)

四、质量分析与反馈

1、对期中、期末考试(查)采取三级质量分析,由教师对所教班级学生情况进行定量分析,并填写情况分析表。

2、备课组长汇总整个年级此门学科的情况,并完整填写年级情况分析表。

3、教研组长对本学科做全面质量分析,提出相应的改进措施,撰写质量分析报告,并在大组内进行反馈。

4、由校长室通过计算标准差等科学统计方法对教师教学质量进行监控和评定。

5、所有考试(查)、校监控试卷和三级质量分析的材料及时交资料室做好存档工作。

 

*对教代表的回复:由于牵涉到个人隐私问题,此项无法根据教代表的提议修改,将监考老师的手机收到校长室不是很妥当。
